AccordantCare Complex Conditions Nurse Care Management

If you or an eligible family member are enrolled in a Baker Hughes medical plan, you have access to AccordantCare through CVS at no additional cost. AccordantCare is a specialized nurse care management program designed to support members living with complex chronic conditions. AccordantCare provides personalized nurse support for members living with complex conditions, helping them better manage their health, access resources, and navigate their care. The program supports eligible conditions including Multiple Sclerosis (MS), Crohn’s Disease, Myasthenia Gravis (MG), ALS, Juvenile Idiopathic Arthritis (JIA), Cystic Fibrosis (CF), Parkinson’s Disease, and more.

• 24/7 access to specialized nurses
• Personalized care and condition-specific guidance.
• Help with medications, symptoms, and emotional support.
• Resources for caregivers and community services.
