Additional Health Resources

Resources and support to improve health care delivery for certain chronic conditions, including bariatric and transplant surgery and pregnancy.

If you enroll in a UHC Medical plan, you have access to the following health resources.

Health Management Program

The UHC Health Management Program is a voluntary program that helps you and your physician improve health care delivery for chronic conditions, such as:

  • Asthma
  • Chronic obstructive pulmonary disease (COPD)
  • Congestive heart failure (CHF)
  • Coronary artery disease (CAD)
  • Diabetes
  • Lower back pain/musculoskeletal problems

Products and services

The Health Management Program offers the following products and services:

  • Quality of life and self-care enhancement resources
  • Assigned personal nurse depending on the care you need
  • Educational and self-care literature mailed directly to you

UHC’s Centers of Excellence

UHC has identified several medical facilities that provide superior specialty services called Centers of Excellence (COEs). Using a COE for certain medical procedures can help you achieve a better health outcome and lower out-of-pocket costs.

You and your covered family members have access to COEs for:

  • Bariatric surgery
  • Cancer treatment and support
  • Congenital heart disease
  • Kidney diseases and conditions
  • Orthopedic conditions

Maternity Support Program (MSP)

If you are pregnant and covered under a Baker Hughes Medical plan, you are eligible (and encouraged) to participate in the UnitedHealthcare (UHC) Maternity Support Program at no cost. It provides a personalized approach that places resources at your fingertips before, during and after your pregnancy. Your spouse may also participate in the program if he or she is enrolled in your Medical plan. Members who enroll in MSP are eligible for rewards through Energize.

Get healthier with Rally

UHC Medical plan participants and their covered dependents have access to Rally, a UHC wellness resource available at no additional cost to you. Rally can help you get healthier, one step at a time. Use Rally to learn how to make simple changes to your daily routine, set smart goals and stay on target. 

Advocate4Me

Managing your health plan benefits and your health isn’t always easy. UHC has a team of people dedicated to helping you. From understanding your claims to estimating costs ahead of time, they can help. Here are examples of questions you can get help with:

  • Is this treatment covered?
  • How much will I have to pay for a test my doctor wants me to get?
  • What does this charge mean on my bill? And why is it this amount?
  • Can you help explain my benefits and what I need to do? 
  • If I need to find a new doctor, can you help me?

NEW: Bend Health – Part of the Optum Behavioral Network

Bend Health, part of the Optum Behavioral Network, provides online mental healthcare for children ages 1-17, without the wait times. Licensed mental healthcare providers are available in every state.

Bend Health has care plans designed for general and specific concerns. Bend Health can help with:

  • Stress, depression, worries, and anxiety
  • Trauma
  • Sleep issues
  • Anger issues
  • Parenting concerns and sibling rivalry

One Pass Select Fitness and Well-being Program - Effective March 1, 2024

One Pass Select is a subscription based fitness and well-being program that supports a healthier lifestyle. You have access to thousands of gyms and online classes with:

  • No long-term contracts or annual gym registration fees
  • The ability to use locations nationwide
  • The option to change tiers monthly
  • A grocery delivery subscription

See below for the full member benefits. One Pass Select Fitness program fees paid can be used for the Baker Hughes Fitness Reimbursement.

CVS/caremark CareManagement Programs

Transform Diabetes Care

CVS/caremark pharmacy plan participants and their dependents have access to the Transform Diabetes Care program, at no additional cost to you. The Transform Diabetes Management Care program supports you and your dependents with a diabetes diagnosis and help you manage your care.  

You can get a customized care plan that includes:

  • Blood glucose monitoring
  • Medication review to help make sure you’re taking the right medications
  • Pharmacist counseling in person or by phone
  • Consultations at CVS HealthHUB or MinuteClinic locations
AccordantCare Complex Conditions Nurse Care Management

AccordantCare is a specialized program that gives members the support they need to manage their complex condition, find helpful information, and stay as healthy as possible. AccordantCare is offered to members with complex conditions such as Multiple Sclerosis, Crohn’s Disease, Myasthenia Gravis, ALS, JIA, Cystic Fibrosis, and Parkinson’s Disease.
