The Conditon Management Resources Program is a voluntary program that helps you and your physician improve health care delivery for chronic conditions, such as:

The Condition Management Resources Program offers the following products and services:

UHC’s Centers of Excellence

UHC has identified several medical facilities that provide superior specialty services called Centers of Excellence (COEs). Using a COE for certain medical procedures can help you achieve a better health outcome and lower out-of-pocket costs.

You and your covered family members have access to COEs for:

• Bariatric surgery
• Cancer treatment and support
• Congenital heart disease
• Kidney diseases and conditions
• Orthopedic conditions

Maven Maternity

Whether you’re thinking about having a baby or have one on the way, the Maven Maternity Program is here to help through every step of the way.

The Maven Maternity program offers:

• 24/7 virtual consultation as needed with 30+ subject matter experts, including doulas, lactation consultants, nutritionists, and more
• A dedicated Maven Advocate to help members navigate the program
• Access to educational articles, drop-in community groups, and classes

Advocate4Me

Managing your health plan benefits and your health isn’t always easy. UHC has a team of people dedicated to helping you. From understanding your claims to estimating costs ahead of time, they can help. Here are examples of questions you can get help with:

• Is this treatment covered?
• How much will I have to pay for a test my doctor wants me to get?
• What does this charge mean on my bill? And why is it this amount?
• Can you help explain my benefits and what I need to do? 
• If I need to find a new doctor, can you help me?

Bend Health, part of the Optum Behavioral Network, provides online mental healthcare for children ages 1-17, without the wait times. Licensed mental healthcare providers are available in every state.

Transform Diabetes Care

CVS Transform Diabetes Care is a personalized, no-cost program designed to help CVS plan participants and their dependents manage their diabetes more effectively. It combines connected health tools, expert support, and digital resources to improve outcomes and simplify care.

You can get a customized care plan that includes:

AccordantCare Complex Conditions Nurse Care Management

AccordantCare is a specialized program that gives members the support they need to manage their complex condition, find helpful information, and stay as healthy as possible. AccordantCare is offered to members with complex conditions such as Multiple Sclerosis, Crohn’s Disease, Myasthenia Gravis, ALS, JIA, Cystic Fibrosis, and Parkinson’s Disease.
