The UHC Condition Management Resources Program is a voluntary program that helps you and your physician improve health care delivery for chronic conditions, such as:

• Asthma
• Chronic obstructive pulmonary disease (COPD)
• Congestive heart failure (CHF)
• Coronary artery disease (CAD)
• Diabetes

The Condition Management Resources Program offers the following products and services:

• Quality of life and self-care enhancement resources
• Assigned personal nurse depending on the care you need
• Educational and self-care literature mailed directly to you

UHC’s Centers of Excellence

UHC has identified several medical facilities that provide superior specialty services called Centers of Excellence (COEs). Using a COE for certain medical procedures can help you achieve a better health outcome and lower out-of-pocket costs.

You and your covered family members have access to COEs for:

• Cancer treatment and support
• Congenital heart disease
• Kidney diseases and conditions
• Orthopedic conditions
